CMS has recently updated its requirements for support letters. All letters must now:
• Be submitted on the facility’s official letterhead
• Include specific required language
• Must be signed by the SNF administrator
A template is attached below for your convenience. Please copy the template onto your facility’s letterhead, sign and date it, and email it to us (mw@goldenyears.co) at your earliest convenience.

If you do not receive a confirmation from Michael that he received your letter of support - it has not been received (make sure you have the correct email address).
