Golden Years, a world-class wellness company, invites you to participate in a free CMS CMP grant opportunity: The Golden Years Strength & Mindfulness Training Program, fall prevention through activity.
You are invited to participate in the CMS-funded Golden Years' Fall Prevention Program, designed specifically for nursing home residents. Through daily chair-based movement, mindfulness, "breathing-work", and wellness education, the program supports resident well-being, reduces falls through activity, and improves QoL.
Since February 2025, the program has been implemented across 61 facilities in North Carolina.
Because this is a CMS grant program, there is no cost to participating facilities.

At the heart of the program is a 30-minute virtual class - delivered as an Activity - offered seven days a week. Each session combines:
The classes have been specifically designed for nursing home residents by clinical and mindful movement experts. Sessions are intentionally safe, accessible, engaging, and adaptable for varying ability levels.
Facilities also have access to a 15-minute pre-class screen featuring music and a countdown timer to help residents settle in before class begins.

Distributed monthly, the newsletter includes evidence-based wellness tips and practical content that facilities can share with residents and staff.
Quarterly webinars explore the science and methodology behind the program and provide additional education for participating facilities.

Golden Years has 25 years of experience creating mindfulness and wellbeing programming. We have a network of highly experienced and vetted wellness professionals, and are working alongside grant partner, Rick Kral, DNP, LNHA, CGCP, NEA-BC, CDP, CPPS and geriatrician, Claire Davenport, MD, MS, to bring our programming to your sites.
The program is free of charge to facilities and residents.
Your facility qualifies if they have a CMS certification number.
No, Golden Years will apply for the grant on the facilities’ behalf. We are
handling the majority of the administrative work required, and our grant partner
holds a DNP.
The grant is applicable for three years. We're confident we'll be able to secure
another grant in three years, and that the program will run continuously.

Yes, as part of the CMS grant program, participating facilities are asked to track up to 10 residents participating in the program. (While more residents are welcome to participate, only up to 10 need to be formally tracked.) The program measures falls, falls with injury, and resident satisfaction. Baseline data is collected at program start, with follow-up collected six months later. Facilities may use our templates or their own internal tracking systems. Data may be emailed or telephoned in.
Yes, anyone can participate, but we only track SNF residents, and only up to 10.
No, the Golden Years' program is designed to integrate seamlessly into each facility’s existing monthly activities calendar. Some facilities attend the daily class once or twice per week, while others attend six times a month. Facilities are encouraged to use the daily classes and resource library in whatever way best supports their residents.
We will host a Zoom orientation with Rick and Michael, and a recording of the session will be shared afterward for anyone unable to attend live.
When programming begins, participants will receive two links: one for the daily class and one for the resource library. Access is simple—there is nothing to download and no passwords or accounts to create. Just click the link to join the class or access the library. The links will also be available on the library website.
We understand that resident participation may fluctuate over time, and that is completely expected. For example, a facility may begin with six participating residents and have four residents included in its six-month data submission. Participation may then increase to five or eight residents, or decrease to three residents, by the next reporting period. These changes are normal and will not affect participation in the program.
